Assura has now merged with PHP in a transformational combination to create a portfolio of critical healthcare infrastructure assets valued at approximately £6 billion. Whilst the two businesses integrate operations Assura customers can continue to log maintenance jobs via the portal and your main contacts at the company remain unchanged.

Dementia support scheme wins our funding

A Hampshire scheme to support people with dementia and their families has won a share of our grant funding.

GPs from the Cowplain Family Practice submitted the Mission Dementia project for one of our Healthy Communities grants. Mission Dementia, which is based at nearby Denmead Practice, works with patients across Waterlooville on challenges such as connecting people with local sources of support, creating individual information packs about care plans and Lasting Power of Attorney documents, so that patients don’t have to answer the same questions for many different agencies, and processes to help ensure good information is quickly available to police if someone with dementia goes missing. The new funding is set to help expand the service for more families.

In its nomination, Cowplain Family Practice said: “We know from research that people diagnosed with dementia and their families need the same help as those diagnosed with other chronic diseases: support to come to terms with the diagnosis and to live well with it.

 “General practice is the ideal location, as we’re close to and familiar with our patients. This project helps to give patients a sense of being in a community where they feel safe and valued.”

Mission Dementia’s Dementia Advisor, Kim Babcock, said: “Bringing people together to share their experiences and develop services around what they want benefits patients, carers and GP practices.

 “The additional funding will provide an opportunity to share our learning so far and engage more people to play an active role in their care.”
